Loss making calls

I keep on getting loss making calls, but only on certain calls, i have fixed my tariffs already and must say my loss making calls are much less.
the calls are very inconsistent, i will have one user that makes 50 calls a day, 45 of these calls will be ok, but the other will register as loss making calls.
How do i fix this error?

Disallow loss calls in Device settings.
